The IP Justification form is a requirement from ARIN, which is regularly audited. Each form submitted will be manually reviewed for accuracy. You must provide valid data to receive additional IP allocations.

1) IP: you have to replace all existing IPs which is with the server IPs that you got.
2) Customer: Should be your name or the name who is responsible for these Additional IPs.
3) Usage: Let us know the usage of your existing IPs that are with your server right now. For what purpose will you use IPs?
4) Domain/Hostname: You should have a qualified domain name or any hostname that is related to you or your company.
Remember that your subnet is xxx IPs, and you have to divide all xxx IPs into the IP-based Virtual Hosting option and the Other (specify) option.
1) IP-based Virtual Hosting: You need to tell us how many IPs will be used for Virtual Hosting.
2) Other (specify): You need to tell us how many IPs will be used for Other (specify)
3) SSL Virtual Hosting: If you will use IPs for SSL Virtual Hosting, you need to tell us how many IPs.
1) Reason for Using IP-based Virtual Hosting: You have to tell us a valid reason in detail for which purpose or what your goal is for the usage of IPs.
